With this form you can send the application attachments to TUAS Admission Services if you could not submit the documents on the application form at Studyinfo. We recommend that all required certificates are attached on the application form at Studyinfo as early as possible. Some certificates will be checked only from those admitted as students.

Document that prove the legal grounds for your non-attendance, when you need time off your studies to take care of a child in regard to the child’s birth or adoption (14.1.2022/52)

Document that prove the legal grounds for your non-attendance, when you cannot begin studies due to being incapacitated by personal illness or injury

Document that prove the needed visual capacity for the students of Biomedical Laboratory Sciences (studied only in Finnish)
